Dentures, implant dentures or All-on-4?

Whether you are choosing for yourself or helping a parent decide, it helps to see the three main ways of replacing a full set of teeth side by side. Here is how they differ in comfort, cleaning and cost, explained in plain English.

The three options

  • Denture: rests on the gums, no implants
  • Implant denture: clips onto 2–4 implants
  • All-on-4: fixed teeth on four implants
  • All-on-4 from £6,495 per arch
  • Written, itemised quote before you decide

Starting with the basics

When most or all of the teeth in one jaw are missing, there are three main ways forward. A conventional denture is a removable set of teeth that rests on the gums. An implant-retained denture, often called an overdenture, is similar but clips onto a few implants so it does not slip. All-on-4 is a full arch of teeth fixed onto four implants, so it does not come out at home at all. None of them is right for everyone, and many families find it easier to decide once they can picture the difference.

How they compare

Conventional denture Clip-on implant denture All-on-4 fixed teeth
Does it come out? Yes, taken out daily Yes, unclipped daily No, fixed in place
Daily cleaning Brush and soak the denture; clean the gums Brush the denture and around the implant attachments Brush in place and clean underneath the bridge
Covers the palate (upper)? Usually yes Sometimes, with a smaller plate No
Implants needed None Usually 2–4 lower; often more upper Four per arch
Cost level Lowest; quoted after an examination Quoted individually after your scan From £6,495 per arch

Conventional dentures may be available on the NHS if you are eligible. Implants are very rarely available on the NHS, so implant dentures and All-on-4 are provided privately.

A checklist to help you choose

  1. How much does movement bother you?
If a loose lower denture is the main worry, clipping it onto implants can make a big difference.
  2. Would you rather take teeth out at night or keep them in?
Some people like the routine of removing a denture; others want teeth that stay put. All-on-4 stays in.
  3. Is surgery an option for you?
Implants involve a minor operation under local anaesthetic. General health, medicines, smoking and gum disease can affect healing and suitability, and we assess them individually.
  4. Who will help with cleaning?
For an older relative, a removable denture can be easier for a carer to clean. We can talk this through together.
  5. What budget feels comfortable?
Dentures cost the least upfront, All-on-4 the most. Finance through Tabeo can help spread the cost of private treatment.

Good to know about each

Dentures

No surgery and a straightforward process. Upper dentures often feel secure; lower ones can move more because there is less to hold them. They need relining or remaking as gums change.

Implant dentures

A popular step up for people whose lower denture never feels steady. Sometimes an existing denture can be adapted to clip on. The small clips wear and are replaced from time to time.

All-on-4

A fixed full arch with no plate. The back implants are angled to make use of the available bone. It needs good cleaning under the bridge and regular hygiene visits.

Bone and gums

Bone can shrink after teeth are lost. Implants put load back into it. If extra bone is needed, we explain the options, which may include grafting, and include it in your written quote.

Questions families ask about full-arch options

My mum’s denture keeps slipping. Is she too old for implants?

Age on its own is rarely the issue. Her general health, her bone and how easily the area can be kept clean matter more. We are happy for you to come to the consultation with her.

Can we try a denture first and think about implants later?

Yes. Plenty of people start with a denture and add implants later. A new denture can sometimes be made so it can be adapted to clip onto implants.

Is All-on-4 the same as an implant denture?

No. An implant denture still comes out for cleaning. An All-on-4 bridge is fixed to the implants and only your dentist removes it.

Will the treatment hurt?

Implants are placed under local anaesthetic, so you should not feel pain during the procedure. Some swelling and tenderness afterwards is normal. We do not offer sedation.

How do we look after a fixed All-on-4 bridge at home?

Brush it twice a day like natural teeth, then clean underneath with interdental brushes, special floss or a water flosser. We show you and any carer how to do this, and regular hygiene visits clean the areas that are hard to reach.

Can we have a temporary set while healing?

Yes, we plan this so you are not left without teeth. With All-on-4, a temporary fixed bridge is often fitted soon after the implants are placed, if suitable.
